Going to the doctor used to be a hassle for Judith Gonsalves and her husband, Alexander, of Whitman. They hope it will now be more like one- stop shopping.
“Before, I’d go to Abington for my primary care doctor, get my X-rays in Brockton or Whitman, my mammogram in Abington and go back to Brockton to see other specialists,” she said. “It was the same for my husband.”
Last weekOn Tuesday, Gonsalves had her first appointment at her primary care doctor’s new offices at Compass Medical, which recently moved to the new Southeast Medical Center in East Bridgewater, just over the Whitman town line, in late July.
Once she made it through the temporary road work and detour at the entrance, things moved smoothly.
Southeast Medical is a brand new, 70,000- square-foot office building that is much larger and more modern than Compass Medical’s former offices in Whitman. Doctors there have It has new affiliations with Boston teaching hospitals. There’s plenty of parking, and Gonsalves can expect to find get most all of her care there, except for emergencies.
If Gonsalves needs her blood drawn or other testing, there’s a full lab on the first floor. She had her annual mammogram there a few weeks ago — it used updated digital scanning — and the radiology suite also has digital X-ray services, the latest 64-slice CT scanner, an MRI machine, bone density testing, ultrasound and will soon have an MRI machine. A radiologist from Brigham & Women’s Hospital in Boston works there full- time, reading tests and consulting with staff.
In Whitman, Compass had nearly 16,000 square feet. Compass now leases 43,000 square feet in the East Bridgewater medical center. The extra room has accommodated job growth: Compass has expanded its work force from 90 people in Whitman to 125 in East Bridgewater.
If either of the Gonsalves or her husband need a cardiologist or several other types of special care, those services are on site too. Many specialized doctors at the facility — including the new vein center — are affiliated with Brigham and Women’s Hospital in Boston. Other specialists are affiliated with the Massachusetts Eye & Ear Infirmary in Boston and Eye Health Services in Quincy. There’s a new urgent care center with evening and Saturday hours and a pharmacy — a branch of Duval’s Pharmacy in Whitman — to fill prescriptions. Orthopedic surgeons are also in the building.
Gonsalves said she felt much more confident after getting her mammogram this year. “The equipment is so new — before it was much older, and you just wondered if you were getting an accurate result,” she said.
“This equipment makes you feel much more confident that you are getting the right diagnosis.”
Located about 30 miles south of Boston, the Southeast Medical Center is not what people might expect to find driving down Route 18 past small roadside stands and local businesses. Just a
A short distance beyond the Peaceful Meadows Farm and Ice Cream Stand in Whitman, with cows in the field, the long, two- story beige-and-white building on its 100-acre site dominates the landscape.
“It’s almost like a hospital — it’s huge,” an elderly woman from Abington said Monday, as she left her doctor’s office. “It’s beautiful, but I’m old and I don’t like change.”
That sentiment didn’t surprise Dr. Jonathan Ellis of Milton, a cardiologist with Compass Medical, who was monitoring a patient’s stress test — another new on-site feature. Despite the need for older patients to adjust to change and also to be patient while some construction is completed, the overwhelming response has been very positive, said Dr. Jonathan Ellis of Milton, a cardiologist with Compass Medical. Ellis said.
“We’ve been drawing in new patients since we opened July 30,” he said.
“They have heard about the convenience and the new equipment.”
Some of the new devices include the 64-slice CT scanner and an MRI machine that will begin operating next month. ” — including some of the most advanced digital imaging devices, like a 64-slice CT scanner, and an MRI machine that will be in place by mid October. “I’ll be able to use those right here in diagnosing patients’ conditions,” he said.

Stay away from Mesa Ridge, Fairways and 7 Oaks. Villas of St. Moritz is a good one though. Good luck.

I just returned from a quick trip to Amsterdam where I was invited to speak at a session called “Health Care Innovation Event”, a conference of the CEOs and other top administrators of many of the Dutch hospitals, along with the Dr. Ab Klink, Minister of Health, Welfare, and Sport. I am a little amazed by this — and no, I am not being falsely modest — but people are very interested in the quality and safety process improvement steps we have taken at BIDMC and also in our efforts at transparency on this blog and on our hospital’s website. I am surprised because I think we are just beginning to tap the potential improvements we can make in this arena, and we consider ourselves as just learning how to do it well. Other hospitals, like those in the Ascension Health system and Cincinnati Children’s Hospital, have been at it for a longer time and with greater results. Nonetheless, it is very nice to have a chance to explain our programs to others around the world and to meet really interesting and thoughtful people. Inevitably, I bring back more ideas than I impart.

After the formal presentations in Amsterdam (at the amazing ING building!), we had break-out sessions during dinner. (They were very accommodating and made my table the sole English-speaking one.) During the sessions, the facilitator at each table was posting the table’s comments in real time on video discussion boards spread around the room. It was like having a dozen simultaneous twitter sites going on! Later, the combined discussion board was used by the MC as a tool for reporting the major conclusions from each table.

By the way, the Netherlands has a very interesting and recently enacted insurance program for the country. This replaced the former system of government insurance. All people are required to have insurance and can purchase it from any of a number of private insurance companies. Some of these companies are for-profit and some are non-profit. There are about four large ones and over a dozen small ones. No company can refuse to provide coverage to any person. The annual cost of insurance is about 1000 to 1200 Euros. The government subsidizes the cost of insurance to people with low incomes. The insurance covers the full range of medical diagnostic and treatment services. You can also buy supplemental insurance to cover things like single rooms in the hospitals, cosmetic surgery, and the like.

Beyond this program of “cure” insurance, there is a separate government program for “care”. This covers long-term care and other parts of the medical care spectrum that are essentially uninsurable.

I was told that the country as a whole spends 45 billion Euros on health care for these two types of coverage, for 16 million people — or about 2800 Euros per year per person. With the “cure” portion amounting to 1000 to 1200 Euros per person, that leaves 1600 to 1800 Euros per year for the “care” portion.

The hospitals are non-profit, like most in the US. Interestingly, payment for services rendered is not generally based on the kind of diagnosis specific (”DRG”) system we have in the US, although there is a movement in that direction. There is actually very little documentation — for billing purposes — of the services rendered. Either doctors are on salary or the billing is based on a simple count of those patients coming through. That’s one way to reduce administrative costs. On the other hand, as was pointed out at our meeting, this means that there is very little accountability for the choice and efficacy of the clinical procedures actually undertaken. A person at our table suggested that Holland should move more to the US reimbursement system to hold providers more accountable. This was met with mixed reactions, as people understood that this would add administrative costs to the system.

I was just at Methodist, I believe they use a Flavia machine, here is the link for at at home version. There are other sites that sell it, but this seems to be one of the better priced ones:
